Yes, you can still walk into your local Meijer store to request a donation. A Customer Service team member will help you complete this online form or enter the donation request on your behalf. Note: To ensure the accuracy of your information, we strongly encourage you to complete the application.
Please submit the request for your event to a single Meijer location. To help ensure we can support many organizations and causes throughout our six-state footprint, it is our policy to provide only one donation per event.
Please allow 60 days for review from the date you apply. When you apply for a donation, you will receive a confirmation number in your email and can log back into the system to check for updates. Once the review process is complete, you will receive an update via email or phone.
You will need to upload a current and signed W9 form and any other documentation supporting your request. This could be a letter, sponsorship proposal, impact report, etc.
As a school entity, you will need to upload a current and signed W9 and provide your National Center of Education Statistics (NCES) code. If you are unsure what that code is, there is information in the form to help.
You can log back into this system to check the status of your request. You will also receive an email notification once the review process is complete. If it has been more than 60 days since you submitted your request, you may contact the Meijer store where you applied to check on the status.

Yes, an organization can have multiple contact names associated with their organization. To be a contact, the person must be a representative of that organization and have a copy of the organization’s current W9.
No, the donation support request form is a separate program and not tied to any other Meijer system, including mPerks.
